We will need your assistance in order to comply with possible insurance requirements that might affect your policy. Please provide a current insurance card so that our office can keep a copy on file.

FOR INSURANCE PLANS THAT REQUIRE REFERRALS OR AUTHORIZATION- you must provide either at the time of your office visit. Otherwise, you will be expected to reschedule your appointment. This is your insurance requirement and, therefore, your responsibility. Please call your insurance company for plan details. If you multiple insurance plans, you may need multiple referrals and/or authorizations.

Please remember that insurance is considered a method of assisting the patient with medical expenses and is not a substitute for payment. Payment is expected at the time of service. We collect co-payment at the time of visit. For your convenience we accept cash, check and major credit cards.
